Breast MRI and Computer-Aided Evaluation Findings of Intraductal Papilloma

Abstract
Purpose: The purpose of this study was to describe the findings of intraductal papillomas in the contralateral breast of breast cancer patients on breast MRI and computer-aided evaluation program (CAE).

Materials and Methods: Between January 2007 and December 2008, of the patients with breast cancer who underwent bilateral dynamic contrast enhanced breast MR examinations prior to surgery, 10 patients(mean age 46 years, range 40-57 years) were proved to have an intraductal papilloma in the contralateral breast through US-guided needle localization and excision. Two radiologists retrospectively analyzed the findings of breast MRI and CAE in consensus. Morphology and kinetic features were analyzed according to the Breast Imaging Reporting and Data System. Proportion of delayed enhancing pattern including washout, persistent, plateau components on CAE were analyzed.

Results: Mean lesion size was 1.0¡¾0.7 cm (range 0.5-2.9 cm). Ninety % (9/10) of the lesions were a mass types, non-mass-like enhancement was 10% (1/10). Seventy eight % (7/9) of the masses showed irregular shape and irregular margin, respectively. Forty four% (4/9) of the masses showed homogeneous enhancement and other 44% of the masses showed inhomogeneous enhancement. One lesion showed rim-enhancement. Rapid early enhancement was found in 60% (6/10). On delayed phase, washout pattern was found in 50% (5/10) and plateau pattern was found in 40% (4/10). On CAE, washout component was found in 60% (6/10) and the mean proportion of washout component was 22% (range 3-36%). Plateau component was found in 90% (9/10) and the mean proportion of plateau component was 21% (range 1-47%). One case showed 100% persistent component.

Conclusion: Majority of intraductal papillomas detected in contralateral breast of the breast cancer patients showed irregular shape, irregular margin, and washout or plateau delayed enhancement pattern on breast MRI and CAE.
